Introduction.

Parkinson’s disease (PD) is a progressive neurodegenerative disorder primarily characterised by bradykinesia, rigidity, tremor and postural instability. Among these impairments, postural instability has major functional importance because it contributes to difficulty maintaining balance, impaired mobility, falls and loss of independence.

Retropulsion—an abnormal tendency to lose balance backwards—is a clinically important manifestation of impaired reactive postural control.

The Retropulsion Test, also known as the Pull Test, is a simple clinical method used to assess the patient’s ability to recover from a sudden backward perturbation. The examiner produces a rapid posterior displacement by pulling the patient backwards at the shoulders and observes the corrective stepping response. The test is incorporated into the Movement Disorder Society–Unified Parkinson’s Disease Rating Scale (MDS-UPDRS).

Postural instability is particularly relevant to physiotherapists because balance dysfunction affects activities such as walking, turning, transferring and responding to unexpected disturbances. Therefore, the retropulsion test provides clinically useful information about reactive balance and protective stepping strategies. However, it should not be considered a complete assessment of fall risk, as falls in PD are multifactorial and involve gait, cognition, environmental factors and other components of balance.

2. Relationship Between Parkinson’s Disease and the Retropulsion Test.

Normal balance requires continuous integration of sensory information and rapid generation of appropriate motor responses. When an individual is unexpectedly displaced backwards, the nervous system should produce an automatic reactive postural response, usually involving an effective backward step to restore the body’s centre of mass within the limits of stability.

In Parkinson’s disease, these automatic responses may become impaired. Dysfunction within the neural systems involved in movement and postural regulation can contribute to delayed, reduced or poorly coordinated corrective stepping. Consequently, when a backward perturbation occurs, the patient may take multiple small steps, continue stepping backwards or fail to generate an adequate protective response.

This abnormal backward displacement or difficulty recovering balance is described clinically as retropulsion. The retropulsion test deliberately challenges the patient’s reactive balance system and therefore provides a practical observation of postural instability.

Research suggests that postural instability may already be present in earlier stages of Parkinson’s disease and becomes more pronounced as the disease progresses. Biomechanical studies have demonstrated that individuals with PD can be destabilised by smaller perturbations and may show inefficient postural adjustments compared with healthy individuals.

The test is clinically valuable because it assesses a specific component of balance: the ability to recover from an externally generated backward perturbation. However, a positive test should not be interpreted in isolation. A patient may have difficulties with gait, turning, freezing, cognition or environment which the Pull Test does not fully capture.

3. Procedure of the Retropulsion (Pull) Test.

The test should always take place in a safe, controlled setting by a clinician experienced in protecting the patient.

→Patient preparation

Have the patient adopt a relaxed, erect posture with the feet parallel. Briefly tell them you will check how they regain balance so they are not taken unawares.

→Therapist position

Remain at their back, near enough to catch a fall, but not so close that stepping is prevented. →Initial positioning

Leave an unobstructed path behind them and clear the floor of anything that could hinder backward steps.

→Backward perturbation

Deliver a brisk, purposeful pull on both shoulders. The force must be strong enough to move the centre of mass and require a corrective response.

→Observation

Note the number of corrective steps, their size and effectiveness, whether balance is regained independently, the quality of the postural response and whether examiner assistance is needed. The MDS-UPDRS supplies standardised instructions for this item. Consistency matters: differences in stance width, pull force, pull speed or examiner technique can alter the observed response. Studies of real-world performance have documented considerable variation, especially in the strength and briskness of the pull.

4. Interpretation of the Retropulsion Test.

Interpretation focuses on the patient’s ability to generate an effective corrective postural response.

→Response

→Clinical interpretation

Recovers effectively with 1–2 steps.

Normal or near-normal response

Requires multiple corrective steps.

Impaired reactive postural control

Takes numerous steps but recovers independently

Significant postural instability

Falls unless supported by the examiner.

Severe impairment of postural response

Research comparing variants of the retropulsion test reported that an unexpected shoulder pull performed once, with more than two backward steps, was a useful criterion for identifying an abnormal response. For formal clinical scoring, the MDS-UPDRS Item 3.12 uses a 0–4 rating system, ranging from normal postural stability to severe instability. The score should be based on the standardised descriptors of the MDS-UPDRS rather than simply using a non-standard step count.

An abnormal result indicates impaired reactive balance, but it does not independently diagnose Parkinson’s disease or predict all future falls. Therefore, physiotherapists should combine the result with gait assessment, turning, functional balance, history of falls and other appropriate outcome measures.

5. The Pull Test identifies a possible impairment; a broader physiotherapy examination clarifies how that impairment affects real-life function.

Current physiotherapy evidence supports balance training for people with PD. The American Physical Therapy Association clinical practice guideline strongly recommends balance training to improve postural control, balance, gait, mobility, balance confidence and quality of life.

6. Conclusion and Clinical

The Retropulsion (Pull) Test remains a clinically useful probe of reactive postural stability in Parkinson’s disease. It challenges the capacity to recover from a sudden backward force and reveals the quality of corrective stepping and protective responses.

Its greatest value for physiotherapists lies less in simply labelling a response “abnormal” and more in understanding functional consequences. Multiple steps or the need for assistance should prompt further evaluation of gait, turning, dynamic balance and overall fall risk. The test must be performed with a standardised, safe technique because examiner differences can affect results. An abnormal finding is best treated as one element within a comprehensive physiotherapy assessment rather than a solitary predictor of falls.

The clinical sequence is therefore:

Identify → Assess → Interpret → Intervene → Reassess.

By combining evidence-based balance and functional training with this structured approach, physiotherapists can target postural-control deficits and support people with Parkinson’s disease in maintaining mobility, confidence and independence.

References

Bloem BR, et al.
